School Clinic Registration Form: 2024-25 School Year

Please use this form to register for the following School Programs only:

- Oakland Terrace ES

Tuition is $135 for the seven week program. Space is not reserved until the registration form is complete and payment is received.
Payment should be mailed to or dropped off at: 10615 Brunswick Avenue Kensington, MD 20895. Checks made payable to Burke Basketball.

You can also pay using Zelle. Please use the following email, SPDBasketball@gmail.com. Zelle will identify the account as Burke Basketball LLC.
